All samples sent to Forensic and Scientific Testing, Inc. must be accompanied by a transmittal letter. The transmittal letter must include the following:

• Name and phone number of fire investigator.
• Case number.
• List of evidence and what analysis is needed.
• Description of incident.
• Elapsed time from fire to evidence collection.
• Unique aspects of incident.
